Enrollment Requirements

  1. Child’s official birth certificate (hospital certificates are not accepted)
  2. Child’s complete, validated immunization record (including four-year boosters for Primary-aged students)
  3. Child’s Social Security card 
  4. Driver’s license of parent enrolling child
  5. Proof of residency (current utility bill, lease or rental contract showing parent name and address)
  6. Any legal documents pertaining to the student
*PRE-K, all of the above, plus:

Immunizations up to date for Pre-K including HIB, Pneumococcal (4-yr boosters recommended) and

  1. Proof of income for household (3 months-worth of pay stubs)

***Other helpful information includes a withdrawal form and a most recent report card from child’s previous school
