  • Attached below is our Food Allergy Disclosure Form, which complies with all of the requirements of House Bill 472.  Texas school districts are required to provide a food allergy disclosure form to all students upon their enrollment.  This form should be placed with each student's educational record.  The information may be disclosed to teachers, school counselors, school nurses, cafeteria staff, and other appropriate school personnel who have a legitimate educational interest, in accourdance with the Federal Educational Rights and Privacy Act (FERPA).  For more information on FERPA and student records, please refer to your FL(LEGAL) and FL(LOCAL) policies.

     

    Please note that this form should NOT be placed in the student's health record.  Only documentation from the student's physician of the student's food allergies should be placed in the student's health record.  However, the school nurse may enter notes about the student's possible food allergies in the student's health records, including adding a notation that the parent has notified the school district of the possible food allergy.

     

    Additionally, please be aware that the district's receipt of information regarding a student's food allergies may trigger the need for an evaluation under Section 504.  The information provided in this form may give the district a reason to suspect that a student has a disability that substantially impairs a mafor life activity (for example, the major life activity of eating).

    Breakfast Rate:

    4th -12th $1.80

    Reduced Breakfast Rate:

    4th-12th $0.00

    Paid Lunch Rate:

    4th-12th $2.95

    Reduced Lunch Rate:

    4th-12th $0.40

     

    Breakfast and Lunch meals for all Primary and Elementary students are free for the 2024-2025 school year. 

    The charge limit is $10.00 for all reduced and paid meals only at Intermediate, Junior High, and High School. No charging for a la carte items.

  • Please fill out the cafeteria refund form and email Emily Coward at ecoward@gatesvilleisd.org if you need a refund on your student's lunch account. Please include a current phone number and address. Information will be verified before request if approved.

     

    Reasons for a refund may include:

    • Moved from the district
    • Eligibility statues has changed
    • Student's school day ends before lunch

    Refund request take one to three week to processed.  All checks will be mailed to verified address.

    To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form which can be obtained online at: Program Discrimination Complaint Form, from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
